Project Description
The Mille Lacs Band of Ojibwe, Department of Natural Resources (MLBDNR/E) office will be accepting lump sum proposals for demolition, well abandonment, and septic system closure at 17237 Walleye Road, Onamia, MN 56359.
General Information:
A mandatory site visit is required by all contractors planning to submit a proposal. The site visit will take place on September 15th at 9:00 AM.
Scope of Work:

• Demolition of identified building
• Including all building materials, building contents, and other identified materials
• Abandonment and closure of a septic system which includes 1 tank, 1 mound, and 1 combination tank.
• Abandonment of one identified well.
• Fill to grade area of removal/demolition
• In addition, the contractor is responsible for:
• For securing all required permits and notifications as required by law.
• All fees for such permits and notifications
• Copies of such permits and notifications shall be provide to MLBDNR/E, County, State, and EPA if needed
• Upon completion of work copies of receipts/logs from qualified landfill(s) where all materials were brought must be provided to MLBDNR/E

Required Proposal Content:
All proposals shall include the following:

• Itemized cost for building demolition, well abandonment, septic system closure, filling of area, and any permit/notification costs
• Estimate of days required to full fill scope of work
• Detailed plan of site safety and security during scope of work
  Bidding Notes
  All proposals shall include the following:
• Consulting Firm’s Proposal
• Copy of current Mille Lacs Band Vendor’s License (or a copy of submitted application)
• Copy of current Certificate of Insurance for Commercial Liability of $1 million and Design Professional Liability of $2 million
• W-9
• Copy of an authorized signature sheet if one is not already on file with the Mille Lacs Band
• List of Subcontractors
• Any other information specific to the project as stated in the RFP

Proposals are due on Friday September 29th, 2023 by 3pm.

Please note that all proposals must be submitted via mail to the P.O. Box. FedEx and UPS will not deliver to a P.O. Box and the Onamia post office will not accept hand delivered items. Please plan accordingly to ensure the timely receipt of your proposal submittal.

PROPOSALS NOT SUBMITTED IN THIS MANNER WILL BE REJECTED.
