REPORTS TO: Manager Care Coordination and Clinical Integration
SALARY RANGE: $32,011 - $74,505

SUMMARY:
The Senior Living Manager will work collaboratively across Health and Human Services departments to ensure residents receive the support needed to continue independent living. This person will work collaboratively with the residents to coordinate needed referrals to internal or external departments/agencies.
QUALIFICATIONS:
• Must have, as a minimum, three to five years' experience working with elders.
• Working knowledge of Microsoft Office such as Word, Excel, PowerPoint,
• Excellent verbal and written communication skills.
• Excellent time management and organizational skills.
• Ability to work independently with little or no supervision, work irregular hours including occasional weekends or evenings.
• Team-focused and able to seek collaboration across departments to gain consensus around mutual goals and build meaningful relationships.
• Demonstrated ability to work with the community at large with a positive, optimistic, solutions-oriented attitude. Shares a deep respect for the community, its stakeholders, and the residents that we serve.
• Committed to upholding confidentiality with a firm understanding of HIPAA.
• Must have current valid driver’s license.
• Must be insurable under the Mille Lacs Band Drivers Insurance policy.
• Must pass a pre-employment background, drug and alcohol test.
DUTIES AND RESPONSIBILITIES
• Collaborate with facilities to ensure work orders are completed.
• Work closely with staff of other departments and throughout the organization to improve processes and workflows to create efficiency.
• Be resource for inquiries, and tours of senior living facilities.
• Welcome new residents and review the resident handbook with them. Complete all new resident paperwork.
• Communicate policies and procedures to residents, family members, and visitors.
• Create and maintain an atmosphere of warmth, personal interest, and positive emphasis throughout the building.
• Maintain and grow a positive relationship with the community, service organizations, community hospitals, clinics, home health agencies, other senior living providers.
• Address resident concerns in a timely manner.
• In collaboration with resident, make internal and external referrals as needed.
• Collaborate with residents to bring in programs or speakers of interest.
• Maintaining adherence to all guidelines related to confidentiality.
WORKING CONDITIONS:
• Nature of work is such that incumbent experiences infrequent periods of moderate stress while dealing with clients, family members, and others.
• Hours are typically 8-5 and almost exclusively in doors in a controlled climate area.
• Little threat of personal danger or risk.
