Case Manager DII

The Case Manager position will work across departments to form an internal and external (community) interdisciplinary team to provide case planning and ongoing case management to patients/clients with physical, psychosocial, and spiritual challenges that are preventing them from reaching self-reliance and self-sufficiency, including; however not limited to chronic disease, homelessness, chemical dependency, and mental health diagnosis. The case manager will be accountable for the full patient/client care cycle. Case manager duties include gathering relevant data, planning, implementing, monitoring and evaluating actions required to meet the patient/client holistic care needs with full respect of the patient/client and where they are in their life journey.
• Associates degree or completion of certification program in health care related field. If licensed or certified it must be current to practice in Minnesota.
• Minimum of three years’ experience in care coordination developing multi-disciplinary care plans, self-management goals and referrals to both internal and external resources. Prefer experience in working with at risk individuals and families.
• Demonstrated knowledge of case management principles, healthcare/psychosocial care management.
• Demonstrated ability to document care to support reimbursement.

• Demonstrated ability to provide a positive attitude and respectful, professional customer service; demonstrated ability to provide motivational support to patients/clients.

• Knowledge of and respect for Mille Lacs Band traditions and customs and experience working with Native American communities, culture, customs and cultural law.
• Demonstrated knowledge of HIPAA and regulatory guidance for patient/client confidentiality.

DUTIES AND RESPONSIBILITIES
• Accomplishes patient/client care by working with individuals/families to discuss treatment needs; develop, monitor, and evaluate treatment plans and progress.
• Facilitates interdisciplinary planning through intake interview to determine need for therapeutic medical, psycho-social, and psychiatric evaluations; reviews the established plan of care and need to know information to support assisting the client/patient in meeting treatment objectives, and plans.
• Documents patient/client progress; communicates clients’ progress by conducting interdisciplinary meetings and evaluations at established intervals.
• Supports patients/client by long-range goal setting and support; provide support and assurance there will be a long term contact to communicate with if the need arises.
• Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends.
• Facilitate care and treatment via scheduling, information sharing, and case coordination.
• Works with inter-disciplinary team to adjust treatment plans and goals when barriers are encountered.
• Assists patients/clients to make informed decisions by acting as their advocate regarding their clinical status and treatment options.
• Facilitates client access to community resources, including locating recreational activities, group support, housing, food, clothing, vocational opportunities or services, life skills training, and relevant mental health services.
• Coordinates and monitors services, including comprehensive tracking of client activities in relation to plan of care.

• Nature of work is such those incumbent experiences regular periods of moderate to high stress levels in dealing with client issues.
• Work is primarily in doors in a controlled climate area.
• Hours are typically 8-5, but extra hours may be necessary to meet deadlines and maintain workload.
• Local travel may be required; out of state travel would be rare.
